Request a Mobile Device
Use this service offering to request a university-owned mobile device such as a cell phone, mobile hotspot/MiFi device, or iPad/tablet with cellular service for conducting work activities.
- When the employee has completed the form, the supervisor or other authorizing party will then get an email directing them to Approve or Deny the Request
- If approved, the Unified Communications team will contact the employee and appropriate staff to discuss device needs or confirm shipping arrangements.
Please Note: Mobile Device requests now only need to be performed for initial acquisition, and Do NOT need to be renewed yearly.
