UND owned cellular telephone service is to be used only for conducting University business. State and federal regulations [SBHE Policy Section 611.10] also do not permit this type of activity even if the employee reimburses the University.

Form Instructions

• Complete the University-Owned Mobile Device and Technology Allowance Authorization Form.
• When the employee has completed the form, the supervisor will then get an email directing them to log into HCM using this link.
  • Go to Employee Self Service > Click on the UND box > Click on Employee Forms > Click on Evaluate a Form > Click Search. At this point, the form should appear for their approval.
